I understand and agree:

1. This form is NOT to be used for urgent matters.
2. I am a student covered by the ASPIRE Student Assistance Program or a family member of such a student.
3. This form is only a request and not authorization for services.
4. Services that can be requested via this form are limited to:
1. Self-Help Information
2. Community Resource Referral
3. Legal Consultation
4. Financial Consultation
5. Emotional Support Services
5. The response to my request will be sent to the email address I provide within one business day. If I do not hear from ASPIRE within one business day I should call 1.888.470.1531 to verify that my request was received.
